Team Leader – 35 hrs per week

Main Purpose of Job

To assist the Unit Manager/RGN with the supervision of Healthcare Assistants in providing a range of personal care to Residents within Gavin Astor House. The job holder will ensure the highest standard of care is delivered to Residents and that commercial success is achieved in line with Company objectives and CQC requirements

People Management

• Support the Unit Manager/RGN in leading a team of Healthcare Assistants effectively.
• Demonstrate clear leadership skills when mentoring, coaching and shadowing of Healthcare Assistants whether they are experienced or inexperienced.
• Conduct yourself professionally when dealing and meeting Relatives. Health Professionals and Dignitaries.
• Be involved as key worker/senior carer in the Admission process and settling in period within the first 4 weeks.
• Take part in informal and formal care reviews for the Residents

Operations

• Delivery of personal care to Residents
• Perform clinical tasks to support the Unit Manager/RGN’s and report back, document or communicate in the Carers contact book ie
• Recording of Vital Signs
• Blood Sugar Monitoring
• Venepuncture – if trained
• Performing simple dressings
• Urinalysis
• Flushing of peg tubes
• Ensure the smooth running of the care shift by monitoring the Healthcare Assistants tasks and their documentation and lead effectively the Healthcare Assistant working with you and report back to Unit Manager/RGN
• Be involved in the compiling and completion of Resident Care Plans within 4-6 weeks of admission, informing the Named Nurses of the Residents needs
• Take part in Care Assessments prior to admission when available with the Unit Manager/Registered Nursing Manager
• Be involved in the Induction and Probationary Period of new Healthcare Assistants as a buddy and feedback the progress to the Unit Manager

Communications

• Liaise with relatives and families of Residents in order to provide the best level of individualised care and support.
• Develop and maintain effective working relationships with all relevant bodies including Local Health Authorities, Social Services and Doctors.

General Responsibilities

• The Team Leader must at all times be professional and conduct themselves appropriately when on duty
• Provide feedback to the Unit Manager/RGN of care staff ongoing development and performance to aid in the Supervision and Appraisal Process
• Attend monthly meetings with the Unit Manager/RGN to discuss Resident health issues and care plans
• As one of the Home’s Health and Safety Representatives, you must continue to monitor the Safety of your Unit in particular Infection Control and Universal Precautions supporting the Infection Link Nurses
• Provide witness testimonies and guidance for Healthcare Assistants undergoing their NVQ diploma Pathways
• Be aware of the Mental Capacity Act 2005 & Deprivation of Liberty Safeguards and you part within that requirement
• Where CPD or other in house training is offered or provided, employees have a responsibility to attend and take advantage of this in order to ensure they maintain and develop professional competence. Where required, staff must keep records of such CPD for the purpose of continued registration to practice

• Minimum 3 years experience in the delivery of care / nursing care/domiciliary care
• NVQ in Health & Social Care Level 3
• Proven leadership & people management
• Fundamental understanding of the CQC essential standards

Desirable
• NVQ Level 3 Management or equivalent management qualification
