Location: Dover, Shepway, Deal
Hours: 37.5 per week
Salary: £19,687 - £22,230 per gross annum
Contract: Permanent

The Primary Care Community Link Service, which has been commissioned by Kent County Council Families and Social Care (Adults), Public Health and Kent Clinical Commissioning Groups and is part of the Live Well Kent contract to support adults (17+) in Kent with mental health issues who need advice and guidance to access services in the community.
The aim of the service is to provide advice and guidance with time limited support for individuals with mental health needs and aims to:
Promote recovery – inspire hope, raise expectations and increase confidence.
Community participation – in leisure, employment and education.
Reduce social isolation – increase social networks and opportunities for peer support.
Maximising choice – promoting choice and control.
Access to services – practical and emotional support.
Service user satisfaction – with service delivery and service outcomes.
Direct user-led involvement – in service design, offer and availability.
Diversity – reflecting the diversity of the communities it supports.
Reduce pressure on GPs
The successful candidate is required to work within Porchlight’s policies and procedures, upholding the values and ethos of the organisation. You will need excellent communication and interpersonal skills, the ability to develop effective working relationships with colleagues, clients and agencies and the ability to handle conflicting demands.
You should have experience of supporting people with mental health needs in the community and working with a range of statutory and voluntary agencies.
Due to the wide geographical area covered by this post, the post holder will require a full driving license and the use of a vehicle, subject to the provisions of the Disability Discrimination Act 1995 and Equality Act 2010
