We are pleased to offer an exciting opportunity to join the Patient Services - Community Outreach team: 37.5 hours (plus on-call commitment)

This is a key Community Outreach nursing post within the Patient Services Team at Heart of Kent Hospice. Working closely with other nursing colleagues and members of the clinical team to provide expert palliative care knowledge and skills for all patients referred to the hospice. The post-holder will exercise complex levels of holistic assessment, judgment and decision making in clinical care. Collaboratively working to triage all referrals to ensure streamlined person centred care for our patients, excellence in communication, care and compassion which will be matched with outstanding organisational skills, and regulatory awareness.

Holding a 1st level degree, you will ideally have further palliative care qualifications or with a willingness to undertake further training you will be ready to take on your next challenge. We provide a 7 day a week service and part of this role will involve participating in the out of hour’s on-call rota, and working at weekends or bank holidays. This role will involve visiting within the local community. A driving licence and access to a car will be essential.
