Making it easier for those caring for someone else to continue living their lives to the fullest.

We are currently seeking a Health & Wellbeing Co-ordinator to join our team in Medway. The requirements of the role will include working across all GP surgeries within the 7 Primary Care Networks (PCNs) in Medway with developing support and guidance for Carers within the GP surgeries and among the practice staff. You will be responsible for identifying Carers using the EMISS database and offer Health Checks with signposting to other services.

To be successful you will have experience or working with Adults with advice and guidance either as one-to-one or in group setting, a working knowledge of health and social care and have excellent communication and interpersonal skills. You will also need to be self-motivated and able to work autonomously.

The location for the position is flexible but candidates must be willing to work out of one of Carers First’s main offices and across GP surgeries. You will be required to have a full driving licence and access to a car.
