People are learning to live with long COVID… could you help them?

This is a new role designed to support patients with a long-term COVID health condition that affects their daily living. As a Self-Management Adviser, you will help individuals to understand their condition, self-manage and improve their health and wellbeing.

You’ll work with patients, encouraging them to take an active role in their own healthcare, and monitor and manage their own conditions, treatments and appointments. You’ll support patients to set goals, problem solve and make lifestyle changes, as well as helping them cope with the emotional impact of a long-term condition.

A key aspect of the role is to connect patients with services that can support their needs and aspirations. This could be services that will help them stop smoking, increase physical activity, eat healthily or access counselling.

You’ll also deliver group-based sessions, arranging speakers and facilitating peer support.

Applicants should have relevant experience working in health or social care, a good understanding and knowledge of the individual impact of long-COVID on patients, and experience of facilitating group sessions.

The role is based in Sevenoaks and covers the surrounding area. Car driver essential.
