Are you an organised, self-assured worker who can direct and support others?

This is an exciting opportunity to join the experienced team at Imago, a leading charity providing support and opportunities to people across the South East. As a Team Leader you will manage a team of Community Care Navigators providing a dual service to both carers and older people and adults with complex needs to improve their health and wellbeing.

You will provide effective leadership to the team, as well as managing a small caseload, ensuring the delivery of a high quality service and meeting performance outcomes. You will have responsibility for day-to-day operational planning and decision-making, and work with the senior management team to continually review and develop the service. You will respond to challenging and complex situations with a sensitive and measured response, in line with Imago Safeguarding and Management Policies.

In addition to training, developing and supporting the staff team, there will be opportunities to broaden your own professional knowledge and expertise.

The ability to establish effective relationships with GPs, other health and social care professionals, other partner agencies and voluntary and community providers is key to this role.
