Job Title: Social Prescribing Coordinator – 1 year fixed term contract with possibility to extend depending on funding.
Responsible To: Community Services Manager
Location: Based in Whitstable to cover Herne Bay, Whitstable, Faversham, Canterbury, Ash and surrounding areas.
Job Purpose: To coordinate delivery of the Connect Well Social Prescribing Service
Hours: Part Time – 3 days per week – days to be agreed by line manager
Annual salary: £21,000 pro rata - Mileage at 40p per mile
Annual leave: 25 days per annum, plus Bank Holidays (pro rata if part time). Increases with length of service.

You must have excellent communication skills, with the ability to talk to people on any level in a variety of settings, including 1-2-1 meetings, small and large presentations to organisations and GPs and many other health care professionals This role is extremely proactive and you will be working to agreed targets. Our Community services Manager has won awards for her community engagement and this will be a fantastic opportunity for you to be mentored and developed. You will work with a great close knit team and being a good team player is essential with this friendly bunch.

More about the role:

The role will be responsible for the management and delivery of the Social Prescribing project, providing a referral system for GPs and other Healthcare professionals in the Canterbury and Coastal CCG area (Herne Bay, Canterbury, Faversham, Whitstable, Sandwich & Ash). The role supports clients to access a range of health, social care, welfare advice and other support services. The role will require proactive engagement with healthcare professionals to stimulate referrals into the programme and with community based providers of services and activities. Clients will be supported on an individual basis to identify activities that will improve their health and wellbeing.

A. DELIVERY

• Act as the central point of contact for referrals to Connect Well Social Prescribing service
• Provide information to referred clients, including signposting where appropriate
• Develop and maintain a client database
• Support clients to access volunteer opportunities through brokerage though a face-to-face service, by email and by phone
• Work with health professionals to deliver feedback mechanisms
• Develop a comprehensive wide knowledge of support services in the community to meet different needs including; social isolation, long term health conditions, wellbeing, housing, unemployment, welfare benefits.
• Extend and develop the directory of activities and services by developing and maintaining a central database
• Recruit and support volunteers and champions to work as part of the social prescribing team in the community
• Communicate well with health professionals to increase visibility of the project and to ensure information is up to date and relevant
• Set up and coordinate special events and outreach events in the community locations
• Attend meetings as appropriate

B. MONITORING, REPORTING AND EVALUATION

• Monitor and evaluate the effectiveness of the service against agreed targets
• Maintain systems to keep accurate records relating to the delivery of the Connect Well scheme
• Produce and submit service reports in the agreed format and in accordance with agreed timetables

C. EXTERNAL PARTNERS AND COMMUNICATION

• Maintain effective relationships with Health and Social Care professionals, Information, Advice and Guidance Services, support services, stakeholders and partners
• Maintain good communication with clients and their families/carers
• Recognise people’s needs for different methods of communication and respond accordingly

D. ADDITIONAL RESPONSIBILITIES

• Adhere to Red Zebra’s policies and procedures at all times including Equality and Diversity, Health and Safety, Confidentiality, Lone Working and Safeguarding
• Ensure that the Connect Well service reflects the vision, mission and values of Red Zebra
• Assume responsibility for producing own correspondence, performing and maintaining administrative tasks
• Undertake any other additional tasks as reasonably deemed appropriate by the Social Prescribing Manager

Essential to the role:

• Use of own vehicle and clean driving licence
• Due to the nature of this role an enhanced DBS check will be required
