This is an exciting opportunity to join a new, flagship Department of Health funded project working with older people and adults with multiple needs. Imago is a leading charity providing support and opportunities to people across the South East. You will be responsible for a dedicated area, and following training, will independently manage a caseload and new referrals.

Referrals are received from hospitals and GP’s. You will meet with clients in medical settings or in their own homes to carry out holistic assessments; and together design plans with the aim of reducing hospital readmissions and improving health and wellbeing. The service offers short-term intensive support; building trusted relationships to improve confidence and motivation, encourage personal choices and to support others to access a range of community, health and social care options.

You will respond to challenging and complex situations with a sensitive and measured response, in line with Imago Safeguarding and Lone Working Policies.

In this role you will bridge the gap between primary care professionals and social opportunities, so a knowledge of local services and the ability to build effective networks with cross sector organisations is essential.

Post is subject to references and an enhanced Disclosure and Barring Service check. A full, clean driving licence and access to a vehicle is essential as you will be working in different locations.
