JOB TITLE – (Senior) Continuing Healthcare Assessor
SALARY - £25k-£40k plus benefits pro-rata
HOURS - Full time/part time/bank positions
CONTRACT – Temporary and permanent positions available
LOCATION(S) – Kent

The role:

• To deliver clinical elements of a patient centred holistic assessment of health care needs
• To use and implement pathways and tools underpinning the Department of Health National framework for NHS continuing healthcare and NHS funded nursing care

The candidate will:

• Be NMC Registered and a 6C’s champion
• Have previous experience in evidence based clinical assessment, covering physical or mental health or learning disability
• Possess highly developed assessment and report writing skills; from either review of medical records for retrospective assessment or from patient visits for current assessment
• Be a caring and compassionate team player with effective communication and learning styles
• Bring the ability to use basic computer packages and systems
• Demonstrate experience and understanding of NHS Continuing Healthcare
