Transition Support Worker
Maternity cover - up to 12 months
Salary - Band 3 Equivalent
37 hours per week

We seek an individual with experience of working with young people in a multidisciplinary setting and of family-centred work, to work proactively in offering support, advice, and information to young people using Demelza.

You will have the ability to undertake of 1:1 sessions and or group work with young people and have proven experience of working with diverse community groups.

You will be able to demonstrate excellent organisational and IT skills with ability to re-prioritise a changing workload and willingness to work flexibly to include occasional evenings and weekends.

You will have advanced verbal and written communication skills and the ability to support young people in informed decision making.

An applied awareness of maintaining professional boundaries, effectively advocating on behalf of young people and understanding of issues facing young people and families with a life-limiting condition or complex needs is essential.

Flexibility of working hours and ability to drive and willingness to travel throughout the catchment area of Demelza is required.

You will have the ability to drive a small mini bus type vehicle or be willing to undertake training.

A Dip HE in Youth and Community Work, Dip SW is desirable.
